Basal implants: the possibility of having fixed teeth on the same day

There are many doubts that arise when determining an election on
what type of implant is the most suitable for each case. For the preparation of this article, we have contacted the
Doctor Dario Castro; who is the only expert specialist in immediate loading Basal Surgery in the Canary archipelago, is a dentist specializing in minimally invasive Oral Surgery. He will be able to guide us on the most innovative technique that currently exists; which is none other than the implants called
cortical or basal.
- Dr. Castro, can you guide us in what this new technique called basal or cortical implants consists of?
- Indeed, basal or cortical implants are implants that are anchored in a bone that is never lost, so that extreme atrophies can be solved quickly and without postoperative pain.
It's a proven technique for over 60 years, it's just that it's been taught in very small professional circles since its inception. I first learned it in Italy, from one of its inventors, Prof. Silvano Tramonte, and later perfected it in various European countries, in order to achieve excellence in implantology, which is immediate function and aesthetics.
The same day that the implants are placed, usually without incisions in the gums or stitches, a temporary and in many cases permanent fixed prosthesis is placed.
Patient satisfaction is immediate. This technique is indicated for patients with coagulation problems or heart problems, since normally, as there is minimal trauma, healing is faster than with conventional techniques.
“The same day that the implants are placed, normally without incisions in the gums or stitches, a temporary fixed prosthesis is placed and in many cases a permanent one”
- I would like you to tell me in a few words the difference that exists and the benefits that a patient would obtain from undergoing this technique compared to the traditional implants that we know today.
- The first is that the gums rarely open, the blood supply to the area around the implant is much greater and there is less or no inflammation.
The second is that bone grafts are avoided in 90% of the cases, since the implants are finer and seek the best bone quality, unlike the conventional ones that seek primary stability in the first few millimeters of bone, which It is usually poor in quality.
- What is true in that legend that says that teeth can be put in the same day, or in a maximum of 72 hours? It is really true?
- Not only is it true, but it is usual in our clinic to put fixed teeth in rehabilitations of the entire mouth in 24/48 hours. With the improved digital flow protocol we have managed to rehabilitate from scratch in a record time of 6/8h, that is, on the same day fixed and sometimes permanent teeth are placed, avoiding more surgeries and unnecessary visits.
Aesthetic rejuvenation with basal implants in 8 hours. We have suddenly taken 20 years off the patient, eliminating expression wrinkles and a much more smiling and beautiful face.
- When you say that with basal implants results can be obtained on the same day; Does that mean that if dental pieces have to be extracted, they are replaced immediately?
- One of the benefits of basal implants is that they can be placed "avoiding" infected areas in the tooth itself, being able to be placed even in chronic processes such as cysts, properly cleaning the area, without having to regenerate more than with the tooth itself, which is "processed", to have the best graft that exists, which is one's own.
- Are you telling me that your own tooth can be used as a bone graft? It is safe?
- Not only is it safe, but it considerably reduces inflammation if it exists, since the body does not detect any foreign body. At the same time as the extraction, while the implant is being placed, the tooth is processed according to the TOOTH TRANSFORMER protocol and the bone defect is filled in the same intervention. It can be used with implants in cases where only teeth are to be extracted, for example wisdom teeth. It can even be used for "future" procedures such as bone autograft.
It is also valid for patients who, due to religious or personal reasons, do not want grafts from any source.
“It is also valid for patients who, due to religious or personal reasons, do not want grafts from any source”
- Another thing that draws our attention is that with this technique that you carry out, no bone graft or stitches are necessary and you even state that the inflammation is almost non-existent. Can you explain this to us?
- Curiously, the inflammation depends more on the damage that is done than on the procedure itself. Any cut in the gum generates inflammation. As there are no incisions or stitches, the implant closes the minimum perforation that is made so that there is practically no bleeding. Therefore, patients suffering from coagulation disorders can be treated with this technique.
Bone grafts are not usually necessary as the implants are anchored in the basal bone (innermost), avoiding areas of poor bone quality (outermost).
- Everyone knows that most people when they require a change of many teeth at the same time understand that they must be totally sedated, and this is always an impediment when deciding. What type of anesthesia do you use to perform this technique?
- On rare occasions, conscious sedation is performed, but the entire procedure is usually done under local anesthesia, as it is minimally invasive.
This case of basal implants corresponds to a patient residing in the United Kingdom, who came to our clinic expressly to undergo this intervention and in just 8 hours we achieved extraordinary results. The image shows the before and after.
- We know that patients come not only from the Canary archipelago, but also from other countries to carry out this type of surgery that is so new and so minimally invasive. Is it that in other countries this type of surgery is not carried out so innovative?
- A great technical team is needed, a 3D skull scanner, an intraoral scanner, a computer occlusion program, a 100% digital laboratory and a lot of experience to manage this system, and it is something that is really within the reach of few professionals. Set a goal and persevere until you achieve it.
In other countries this technique is not known; or it is much more expensive, so they decide to come here.
“In other countries this technique is not known; or it is much more expensive, so they decide to come here.”
- And now I'm going to ask you the million dollar question. Does it hurt to undergo this surgery?
- Much less than a dental extraction. Pressure is usually felt in the area for the first few hours, which goes away when the bone adapts to the situation.
- And finally, doctor, what duration and care do people who have had basal implants take?
Prior periodontal treatment must always be done and we recommend following up on the dental cleaning calendar even if the entire mouth is rehabilitated, since everything requires maintenance. We also recommend daily care with a dental irrigator plus brushing after meals. As patients do not have pain or bleeding gums with this technique, they usually respond very well and realize that if they do not take care of it, it will not last forever.
